At Connective Care we want to support your day-to-day successes. Sometimes, you just need a little bit of help. We are driven to improve your outcome by focussing on services that promote:

Aging in place, independence, and life enrichment from a psychosocial and client centered approach.

Connective Care will assist you with your day- to -day life by providing hands on support and connecting you to services that will make your life easier by enhancing all areas of your life as required (social, emotional, physical, spiritual etc). We will meet with you and create an individualized plan.

Services

Aging Consultation services include:

  • Consultation and coaching. Certified Professional Aging Consultant® Certified Dementia Care Provider®
  • Completing a needs assessment and connecting you to the most appropriate services which will meet your needs.
  • Home safety assessment and recommendations along with follow up. Certified Senior Home Safety Specialist™
  • Wellness checks.
  • Arrange for transportation to appointments, social activities etc
  • Assist with medical appointments, in person and via Zoom. If required, we will provide technology to attend virtual appointments and set everything up for you!
  • Accompaniment other appointments to provide advocacy, support, etc
  • Before and after surgery support and planning
  • Transition Planning (for example, when discharged from hospital or moving into a retirement community or a long term care home).
  • Assisting with managing paperwork, making appointments, etc.
